Lions should conduct international activities that illustrate the Third Purpose of the Lions: To create and foster a spirit of understanding among the peoples of the world. Incentive and recognition programs encourage all Lion activities including international relations.

Awards for Lions Clubs
- International Understanding and Cooperation Award (Outstanding Club Patch)
An Outstanding Club Patch award is mailed each year to the district governor for presentation to the one club selected for outstanding participation in International Understanding and Cooperation activities. The governor may seek the advice of the International Relations chairman regarding selection of the award recipient.
- International Club Twinning
The International Club Twinning Program is a reciprocal program between clubs of different countries. Its purpose is to give Lions and Leos of different cultures an opportunity to learn about one another. Upon reporting the first twinning to international headquarters, clubs receive a twinning patch or twinning certificate. Clubs receive no further award or recognition for that twinning relationship.
If the club initiates another international twinning during another year, the club may then apply for another patch or certificate for the second twinning. Whether a club twins once or 20 times during a fiscal year, it still receives only one award. Clubs are advised to limit the number of twinnings in order to develop those twinning relationships.
- Award for Lions Top Ten Youth Exchange Chairpersons
To be eligible, a Lion must actively participate in the Lions International Youth Exchange Program during the fiscal year of nomination.
